A Care Coordinator is trained to provide ethical, reliable, and personalized supervision tailored to meet the recovery plans, and commitments to success for clients throughout the spectrum of recovery. The Care Coordinator will provide continued care assistance from community reintegration following treatment, incarceration or any dysfunctional and debilitating behavioral problems to regain stability in one’s life.
This faith-based, client-centered service includes an ongoing source of connection, helping participants access treatment, recovery support services, and creatively addressing barriers to individual success. It also includes the provision of assessment, care planning, referrals, and monitoring collaboration with clinical services and probation and parole if needed. Recovery services also includes access to supportive housing, educational opportunities, meaningful employment, leisure activities and wellness, family and community social support, parenting education, spiritual engagement, nourishment assistance programs, and any other individualized resources needed to help participants lead a healthy and fulfilling life. Lastly, your Care Coordinator will embrace and celebrate the gifts that you possess as you move forward into a life of recovery.

What if I or my loved one don’t see the need for the level of supervision provided through care coordination? In some cases, the most pressing need for someone in recovery can be met knowing they have a consistent relationship with a mentor while making the transition to a recovery lifestyle. Sober Living Coaching is designed to be client driven, to act as an additional support to ensure the crossroads to recovery are not seen as insurmountable, but manageable.
Visits will be established weekly with follow-up contact dependent on your preferences. In these weekly sessions, you can expect to identify skills and areas of need, receive referrals to available resources, and random drug testing (should you express consent). Length of coaching can vary anywhere from 3 weeks to 3 months. As someone who knows the challenges of recovery, the Sober Living Coach fulfills roles of a mentor, ally and connection out of addiction and into a stronger community and lifestyle.
PEER SUPPORT SPECIALIST.
A Peer Support Specialist is a person who uses his or her lived experience of
recovery from addiction and who models skills and behaviors as outlined in a client treatment plan. The Peer Support Specialist will attend necessary meetings to support the goals and expectations of the client’s treatment plan.
A collaborative approach will ensure client experience and perspective receives top priority. CANDOR KC believes that every client is an expert in their situation and possesses the answers and intrinsic motive to create the life that they want.
Evidence-based, Faith, holistic wellness, client perspective, and goal setting comprise the foundational elements of CANDOR KC services.
