Sober Living Companionship

CARE COORDINATION.

A Care Coordinator is trained to provide ethical, reliable, and personalized supervision tailored to meet the recovery plans, and commitments to success for clients throughout the spectrum of recovery. The Care Coordinator will provide continued care assistance from community reintegration following treatment, incarceration or any dysfunctional and debilitating behavioral problems to regain stability in one’s life.

This faith-based, client-centered service includes an ongoing source of connection, helping participants access treatment, recovery support services, and creatively addressing barriers to individual success. It also includes the provision of assessment, care planning, referrals, and monitoring collaboration with clinical services and probation and parole if needed. Recovery services also includes access to supportive housing, educational opportunities, meaningful employment, leisure activities and wellness, family and community social support, parenting education, spiritual engagement, nourishment assistance programs, and any other individualized resources needed to help participants lead a healthy and fulfilling life. Lastly, your Care Coordinator will embrace and celebrate the gifts that you possess as you move forward into a life of recovery.

SOBER LIVING COACH.

What if I or my loved one don’t see the need for the level of supervision provided through care coordination? In some cases, the most pressing need for someone in recovery can be met knowing they have a consistent relationship with a mentor while making the transition to a recovery lifestyle. Sober Living Coaching is designed to be client driven, to act as an additional support to ensure the crossroads to recovery are not seen as insurmountable, but manageable.

Sober Living Coach

Visits will be established weekly with follow-up contact dependent on your preferences. In these weekly sessions, you can expect to identify skills and areas of need, receive referrals to available resources, and random drug testing (should you express consent). Length of coaching can vary anywhere from 3 weeks to 3 months. As someone who knows the challenges of recovery, the Sober Living Coach fulfills roles of a mentor, ally and connection out of addiction and into a stronger community and lifestyle.

Peer Support Specaist

PEER SUPPORT SPECIALIST.

A Peer Support Specialist is a person who uses his or her lived experience of
recovery from addiction and who models skills and behaviors as outlined in a client treatment plan. The Peer Support Specialist will attend necessary meetings to support the goals and expectations of the client’s treatment plan.

A collaborative approach will ensure client experience and perspective receives top priority. CANDOR Professional Services believes that every client is an expert in their situation and possesses the answers and intrinsic motive to create the life that they want.

Services include: Care Coordination, Peer Support Counseling, Sober Living Coaching and Motivational Speaking.

CANDOR Professional Services' Ladder of Success Workshop Series: The following workshops are offered in 2-hour blocks. They are tailored to the audience requesting them.

  1. Family Cohabitation Intervention.

  2. Interpersonal Development.

  3. Coping with Stress and Anxiety.

  4. Time Management & Financial Literacy.

  5. Substance Abuse Awareness.

  6. How to Motivate Self and Others.

ND RENT HELP. (*)

Provides rental assistance and housing stability help. You may be eligible for up to 12 months of help with your rent through a NEW emergency rental assistance program (ND Rent Help). With higher income limits, more households qualify.

ND Rent Help replaces the Emergency Rent Bridge.

CANDOR'S ND RENT HELP CLINIC. (*)

Candor's vision behind the ND RENT HELP Clinic is the help facilitate a safe working environment and process flow that will allow you to obtain these needed services. We will walk with you through each step of this process which culminates in available sustainable housing. 

Included in this clinic will be access to Candor's Ladder of Success Workshop Series: This series was created to share tips and tools to empower individuals. This clinic is designed with the vision to help end chronic homelessness. We will address any and all barriers moving forward towards an individual's success. Candor is well established in the community with access to resources such as food, education, clothing, housing, employment, social and spiritual support. Individuals can refer themselves or receive referrals through any community-based entity that they currently engage with. 

This is a FREE clinic that is supported by the state's ND RENT HELP Housing Facilitator Contract. Each clinic will service up to 15 individuals at once time. The location currently is at the Bismarck Veterans Memorial Public Library, 515 N 5th St, Bismarck, ND 58501. Individuals interested in participating must be registered by the end of the 1st week of the month. The clinic will meet on the last 3 Tuesdays of the month at 4PM.

 

You will receive a confirmation email or text inviting you to show up at the library with your requested documents. Be prepared for each session to stay at least 1.5 hours. The first session will be used to complete your application. The remaining sessions of the clinic will be educational workshops.

  • $125 - Assessment/Consultation

(Self Referred, this will be your assessment rate).

  • $60 - Group Rate Assessment/Consultation 

(if you have attended a group session and are self referring or being referred from your group’s administrator this will be your assessment rate).

  • $60 - Per Additional hour.
